Paul B Roache MD and ShoulderMD specialize in world class surgical and non-surgical treatments of rotator cuff injuries and shoulder instability. We are the only USA center for Superior Capsule reconstruction with Fascia Lata autograft as described Dr Mihata for patients with massive and irreparable rotator cuff tears.
New patient requests go through an initial medical review to ensure your problem can be well treated by Dr Roache and the ShouderMD team.
Once admitted to the practice care begins:
- Your Patient coordinator will contact you to complete the intake for your consultation.
- Once you complete your consultation with renowned Shoulder Specialist Dr Roache your treatment plan is begun.
- Surgical patients will be followed by the surgical team as you prepare for surgery.
- Drivers License or valid ID
- Valid email address for completion of New Patient Questionnaire and Shoulder Patient Questionnaire
- Insurance information
- Referral letter (if required)
- Reports, X-rays, MRIs, and any other relevant information
- List of medications
